receipt preprinted with the

- The
completed 360 warranty extension
card. (If the card has been sent
separately, please include a copy of
the card.)
- The
completed 360 trade-up certificate.

main body (no hoses) of your old

It is
not necessary to take any
extraordinary precautions when
packaging your trade-in cleaner. A
broken cleaner will not affect your
trade-up eligibility. We suggest
putting items 1, 2, & 3 in an
envelope and taping it to the old
cleaner as an added precaution. We
recommend that you send the cleaner
UPS and save all postal
receipts in case a trace becomes
necessary.
Please
note: Polaris needs to receive these
items no later than September 30,
2009. Polaris will
mail your check within one week of
receiving all four items.
